Faulty signaling and signalling, along with multiple faults in the telecommunication (S&T) department are the cause of this horrific accident. The accident could have been avoided if the driver had been warned earlier by displaying the red flag.
Commission of Railway Safety (CRS) submitted independent investigation report to Railway Board. Where it emerged that the S&T personnel could have taken action despite the fault in the signaling system.
If two parallel tracks were added, the switches would “re-behave” so they could be reworked properly.
S&T staff has shown negligence in that regard. The station manager of Bahanaga Bazar, the site of the accident, said that these technical issues can be fixed.
The report also cited non-supply of station-specific diagrams for replacement of electric hoist barrier at level crossing gate 94 of Bahanaga Bazar station as another reason for the “misstep” of the approved circuit diagram.
